
Appointments
Our office is open for appointment scheduling,
prescription refills, clinical and administrative
questions, etc.
MONDAY–FRIDAY 8:30 AM to 5:00 PM
Appointments are required; however, same day
appointments are possible. If you have an
emergency situation, please call our office
before you come.
We make a sincere attempt to honor your
appointment time; however, the nature of our
specialty sometimes makes delays unavoidable.
A Team Approach To Care
Our practice adheres to the team approach to
patient care and will provide status reports to
your physician to communicate your progress
throughout the treatment process.
We participate with most major health
insurance carriers. If you are not certain if we
accept your insurance, please call our office to
confirm our participation with your carrier.
We will file the required insurance claims to
your insurance carrier; however, you are
financially responsible for non-payment of any
fees not covered by your insurance.
We recommend you become familiar with your
health insurance policy, coverage, limitations
and regulations concerning referrals and pre
authorizations.
Payment For Services
Payment is due at the time of your visit. If we
participate in your health care plan,we will collect
your co-payment or deductible at that time.
For your convenience we accept Visa and
MasterCard, as well as cash and personal checks.
Telephone Calls
If you have questions or concerns about your
course of treatment, please speak with our staff
during regular office hours.
If they are unable to help you, one of the doctors
or physician assistants will return your call at his
earliest convenience.
Prescription Refills
Please ask for prescription refills at your office visit.
Prescriptions may be renewed by telephone during
regular office hours.Prescriptions for pain medication
will not be renewed at night or on weekends.
Emergencies
A physician is available for emergencies at all
times.During office hours, please call our office;
after office hours and on weekends, please call our
office and your message will be given to the
doctor on call by the answering service.
On Your first Visit With
Us
Please remember to:
- Bring any x-rays and prior treatment
records from your referring physician. It is the patient’s
responsibility to obtain these records.
- Bring a list of your medications.
- Bring your insurance card or written
authorization for workers’ compensation
treatment.
